Contract Orthodontist definition
Contract Orthodontist means a Dentist who specializes in orthodontics, and who has agreed to provide Benefits to Enrollees under this Contract.
Examples of Contract Orthodontist in a sentence
No additional charge to the Enrollee is permitted except for services provided by an orthodontist other than the original treating Contract Orthodontist or dental office.
The Contract Orthodontist will prorate the amount over the number of months to completion of the treatment.
The Enrollee will make payments based on an arrangement with the Contract Orthodontist.
