Eligible Patients definition

Eligible Patients means the estimated number of potential Care Plus patients for which a PHO received Care Plus funding according to the Care Plus funding formula.
Eligible Patients means those patients designated as eligible recipients in the gift instrument or other documentation establishing the Fund or, to the extent such designation is not made or is no longer applicable for any reason, patients who demonstrate financial need as determined by the Hospital following examination of available income, assets and such other information as may be required.

  • Those persons who are determined not to be Eligible Patients shall be processed in accordance with Hospitals’ billing and collection policies.

  • Notwithstanding the foregoing, to the extent that such illness, injury or other condition persists for more than one (1) year, Eligible Patients must verify their eligibility for Financial Assistance under this Policy at least annually by providing Hospital with sufficient information to conduct an initial screening as described in Section C.2 (Presumed Eligible Patients) or by submitting an application for Financial Assistance in accordance with Section C.3 (General Application Process).

  • By participating in and accepting any benefits of the Luxe Rebate Program, Eligible Patients agree to, and accept, these Terms and Conditions, as well as any amendments made to them from time to time, and the decisions of Sientra with respect to their interpretation and application which are final and binding in all matters related to the Luxe Rebate Program.

More Definitions of Eligible Patients

Eligible Patients means for each Indication, the number of individuals within the Territory suffering from or afflicted with that Indication, as most recently determined, estimated or reported by: (a) an appropriate agency of the Government of Canada, the World Health Organization or any comparable or successor organization, or any peer-reviewed article in a reputable medical journal, or if such data or information for the Territory is not available, then by (b) pro-rating the number of individuals suffering from or afflicted with that indication in the United States of America, as most recently reported by the Center for Disease Control in Atlanta, Georgia, by the ratio of the general population of the Territory to the general population of the U.S.A.
Eligible Patients means patients who are enrolled in a General Practice associated with the Customer;

  • Eligible pupil means a pupil who has been expelled from school under the mandatory expulsion provisions in section 1311 or 1311a of the revised school code, MCL 380.1311 and 380.1311a, a pupil who has been suspended or expelled from school under a local policy, a pupil who is referred by a court, a pupil who is pregnant or is a parent, a pupil who was previously a dropout, or a pupil who is determined by the district to be at risk of dropping out.
