Evaluation Forms definition

Evaluation Forms. All formal evaluations shall be filed in the employee's file in the Personnel Office, and are available for the employee's inspection by appointment. A copy of the completed evaluation forms shall be given to the employee. The evaluation forms shall be those shown in Exhibit B.

A copy of the completed evaluation form shall be given to the employee. Effective 1/1/2010, all evaluation scales will be changed from the three ratings of Unsatisfactory, Satisfactory, and Outstanding to five ratings of Unsatisfactory, Needs Improvement, Meets Expectations, Exceeds Expectations, and Outstanding. The evaluation form shall be as shown in Appendix D attached and as of 1/1/2010 shall be available electronically.

Examples of Evaluation Forms in a sentence

  • The MSP will evaluate all Authorized User feedback from the Authorized Users Preliminary Technical Evaluation Forms and the Authorized User Interview Evaluation Forms.

  • The evaluation shall also include the evaluation by each unit member within the work area/program, the results of which shall be recorded on a form (See Department Chair Evaluation Forms XX–1 or XX–2 in Form Supplements); provided, however, that any changes made on the form by the Employer shall be preceded by consultation with the Joint Study Committee.

  • Employees shall have access to the Personnel Files and Evaluation Forms before or after working hours.

  • Steps to be followed and factors to be evaluated by supervisors in completing the Performance Evaluation Forms are described on the reverse side of the forms, see Appendix B.

  • For evaluations in the third and fourth years, Student Evaluation Forms shall be administered in a number of sections (classes) taught by the Probationary Unit Member for the semester of evaluation, equated at not less than forty (40) percent of the sections assigned for the semester as selected by the Tenure Review Committee.

  • For each semester in which evaluation is to be conducted in the first and second years, Student Evaluation Forms shall be administered in all sections (classes) taught by the Probationary Unit Member.

  • Steps to be followed and factors to be evaluated by the supervisor/evaluator in completing the Performance Evaluation Forms are described on the reverse side of the forms.

  • Section 19.6.2 Student Evaluation Forms will be distributed to the identified classes by the School ▇▇▇▇ or approved Administrator or designee by the established timelines (C-1) of the semester.

  • The sections and times for which Student Evaluation Forms are to be administered shall be selected by the faculty member and recorded during the Pre-Evaluation Conference with the School ▇▇▇▇ or approved Administrator.

Related to Evaluation Forms

  • Evaluation Criteria means a benchmark, standard or yardstick against which accomplishment, conformance, performance and suitability of an individual, alternative, activity, product or plan is measured to select the best supplier through a competitive process. Criteria may be qualitative or quantitative in nature.

  • Annual Information Form means a document filed by the funds with Canadian securities regulators which provides supplementary information about the funds.

  • Evaluation Team means the team appointed by the City; “Information Meeting” has the meaning set out in section 2.2;

  • Evaluation Report means that document which becomes a part of the employee’s personnel file.

  • Evaluation Period bears the meaning ascribed thereto in Section 7.4(d)(i);
