Extended Health. Cost Shared Deductible: Employee Employer Single Family maximum lifetime benefit for smoking cessation drugs per day hospital benefit (equivalent to Level No reimbursement for ‘over-the-counter drugs’ lifetime benefit NOTE: will be the carrier of the Dental and Extended Health Plan, but Health reserves the right to change carriers. The Union shall be consulted on any proposed amendments or changes with respect to welfare plans and benefits. For the purpose of this Article, for each calendar month for which the Employee receives pay for at least ten (10) days, the Employer shall pay its portion of the premium for the benefit plans as specified in this Article, but not otherwise. However, Employees may obtain benefit continuation by paying, at least two clear months in advance, both the Employer’s and their share of the benefit premiums, subject to the terms of the benefit plans.
