Final Narrative Report definition

Final Narrative Report means the final report the Partner must submit after completion of the Project providing an overview of the achievements of the objectives of the Project for the full duration of the Project and Grant;
Final Narrative Report. The Final Narrative Report is due to HCA on or before February 29, 2016, and will outline completion of the required activities listed above. The ACH Readiness Proposal may be submitted independently or as an attachment to this final report. The following represent the required activities to be described in the final report:  Design and/or early implementation of community or regional initiatives, if applicable.  Participate and provide the appropriate information to inform regional, state and federal evaluation needs.  Participate in a statewide ACH learning network, partnering with the State and the other ACHs to promote shared learning across regions. To accomplish this Scope of Work, the Contractor will:  Focus on recruiting governance leaders representing content experts in multiple healthcare delivery specialties, individuals representing the geographic scope of the region, business leaders, philanthropically-minded individuals interested in improving overall public health and healthcare consumers.  Spend the necessary time to forge working relationships with governing members, staff members, community partners, elected officials, policy makers, and other stakeholder groups.  Establish lines of communication and trust across the broad array of constituencies in order for the OCOH to be successful.  Manage and facilitate meetings of Olympic Community of Health stakeholders, and the Steering Committee.  Organize and facilitate meetings of the OCOH’s subcommittees to ensure completion of deliverables according to established timetables, including meetings of the Governance Subcommittee, Community Health Assessment and Planning Subcommittee (working collaboratively with Awardee epidemiology staff), and Sustainability Subcommittee.  Lead efforts to strengthen stakeholder participation and outreach, including identification of missing stakeholders.  Be the voice of the OCOH governing body in articulating its vision and programs.
