Immediate services. If, during the course of the assessment, the State de- termines that immediate mental health services are required because of the severity and magnitude of the dis- aster, and if State or local resources are insufficient to provide these serv- ices, the State may request and the Re- gional Director, upon determining that State resources are insufficient, may provide funds to the State, separate from the application process for reg- ular program funds (described at para- graph (g) of this section). (1) The application must be sub- mitted to the Regional Director no later than 14 days following the dec- laration of the major disaster. This ap- plication represents the Governor’s agreement and/or certification: (i) That the requirements are beyond the State and local governments’ capa- bilities; (ii) That the program, if approved, will be implemented according to the plan contained in the application ap- proved by the Regional Director; (iii) To maintain close coordination with and provide reports to the Re- gional Director; and (iv) To include mental health dis- aster planning in the State’s emer- gency plan prepared under title II of the ▇▇▇▇▇▇▇▇ Act. (2) The application must include: (i) The geographical areas within the designated disaster area for which serv- ices will be provided; (ii) An estimate of the number of dis- aster victims requiring assistance; (iii) A description of the State and local resources and capabilities, and an explanation of why these resources cannot meet the need; (iv) A description of response activi- ties from the date of the disaster inci- dent to the date of application; (v) A plan of services to be provided to meet the identified needs; and (vi) A detailed budget, showing the cost of proposed services separately from the cost of reimbursement for any eligible services provided prior to ap- plication.

Parents and guardians must assume a responsibility for maintaining this partnership by supporting the school in the provision of education to their children within the scope of School's registration and furthering the spiritual and academic life of their children.

  • Special Services Should the Trust have occasion to request the Adviser to perform services not herein contemplated or to request the Adviser to arrange for the services of others, the Adviser will act for the Trust on behalf of the Fund upon request to the best of its ability, with compensation for the Adviser's services to be agreed upon with respect to each such occasion as it arises.

  • Surgery Services This plan covers surgery services to treat a disease or injury when: • the operation is not experimental or investigational, or cosmetic in nature; • the operation is being performed at the appropriate place of service; and • the physician is licensed to perform the surgery. This plan covers reconstructive surgery and procedures when the services are performed to relieve pain, or to correct or improve bodily function that is impaired as a result of: • a birth defect; • an accidental injury; • a disease; or • a previous covered surgical procedure. Functional indications for surgical correction do not include psychological, psychiatric or emotional reasons. This plan covers the procedures listed below to treat functional impairments. • abdominal wall surgery including panniculectomy (other than an abdominoplasty); • blepharoplasty and ptosis repair; • gastric bypass or gastric banding; • nasal reconstruction and septorhinoplasty; • orthognathic surgery including mandibular and maxillary osteotomy; • reduction mammoplasty; • removal of breast implants; • removal or treatment of proliferative vascular lesions and hemangiomas; • treatment of varicose veins; or • gynecomastia.
