Incomplete/invalid claims definition
Incomplete/invalid claims means claims that are missing critical information (e.g., HICN, procedure code) and therefore do not meet the definition of a claim. Such claims are returned to the provider and may be corrected and resubmitted, but do not have appeal rights. IOM Pub. 100-04, Chapter 1, Section 70.
