To be valid and timely, opt-out requests must be postmarked on or before the last day of the Opt-Out Period (the “Opt-Out Deadline”), which is 28 days before the Final Approval Hearing, and mailed to the address indicated in the Long-form Notice.

Notice shall be provided to Settlement Class Members in three different ways: Mailed Notice; Published Notice; and Long-form Notice on the Settlement Website.
The request must be mailed to the Settlement Administrator at the address provided in the Longform Notice postmarked no later than thirty (30) days prior to the Final Approval Hearing.
