Medical Case Manager definition

Medical Case Manager means a registered nurse or ARNP employed by the Agency to provide nursing consultation and technical assistance to an Area office regarding the medical care of Agency clients.
Medical Case Manager or “MCM” means a health care practitioner employed by the Agency to provide consultation and technical assistance regarding the health and safety of Agency clients.
Medical Case Manager means a person, usually a nurse or social worker, who assists in the planning, coordination, monitoring, and evaluation of medical services for a patient with an emphasis on quality of care, continuity of services, and cost-effectiveness.
