Medical Questionnaire definition

Medical Questionnaire means, with respect to a Referenced Settlement Recipient, the questionnaire concerning medical information, duly completed by or for such Referenced Settlement Recipient and in such form as provided by the Purchaser.
Medical Questionnaire means a series of questions designed to indicate the existence of a medical condition.
Medical Questionnaire means, with respect to a Claimant, the questionnaire concerning medical information, duly completed by or for such Claimant and in such form as the Originator may use from time to time in accordance with the Credit Policy Manual.

Medical Questionnaire means questionnaires provided by us for you to record your Medical History and preferences;
