Medical Questionnaire definition

Medical Questionnaire means, with respect to a Referenced Settlement Recipient, the questionnaire concerning medical information, duly completed by or for such Referenced Settlement Recipient and in such form as provided by the Purchaser.
Medical Questionnaire means a series of questions designed to indicate the existence of a medical condition.
Medical Questionnaire means the medical history questionnaire completed by a registrant of the Illinois Adoption Registry and Information Exchange (Section 18.06 of the Adoption Act).
