Medication regimen review definition

Medication regimen review means an individual resident record review conducted by the con- sultant pharmacist, including, but not limited to, laboratory tests, dietary requirements, physician's and nurse's clinical notes, physician's orders and progress notes, in order to monitor for potentially significant adverse drug reactions, drug-to-drug and drug-food interactions, allergies, contraindica- tions, rationality of therapy, drug use evaluation, and laboratory tests results.
