Per Hour Rate definition

Per Hour Rate means the Anthem Rate that is applicable when payment is derived based on an increment of time multiplied by the Anthem Rate in the applicable fee schedule.
Per Hour Rate means a director’s or employee’s (as applicable) annual total compensation (including salary and bonus) divided by 2,000 hours.
Per Hour Rate means the Wellpoint Rate that is payment based on an increment of time for Covered Services.
