Per Visit Rate definition

Per Visit Rate means the Anthem Rate that is expressed as the all inclusive fixed payment for one outpatient encounter.
Per Visit Rate means the Wellpoint Rate that is the all-inclusive fixed payment for one encounter for Covered Services.
