Personal Information (PI) definition

Personal Information (PI) as used in this policy means either of the following:
Personal Information (PI) means individually identifiable information about an individual collected online, including:
Personal Information (PI) means any information or data that (a) identifies an individual, including by name, signature, address, telephone number or other unique identifier; (b) can be used to identify or authenticate an individual, including passwords, PINs, biometric data, unique identification numbers (e.g., Social Security numbers), answers to security questions or other personal identifiers; (c) is “non-public personal information” as defined in the Gramm-Leach-Bliley Act 15 U.S.C. § 6809(4) or “protected health information” as defined in 45 C.F.R. § 160.103; (d) is an account number or credit card number or debit card number, in combination with any required security code, access code, or password, that would permit access to an individual’s financial account; or (e) is “Personal Information” as defined in The California Consumer Privacy Act of 2018 (Cal. Civ. Code Division 3, Part 4, Title 1.81.5).

  • Information Assets SUBRECIPIENT shall have in place operational policies, procedures, and practices to protect State information assets including those assets used to store or access Personal Health Information (PHI), Personal Information (PI), and any information protected under the Health Insurance Portability and Accountability Act (HIPPA) (i.e., public, confidential, sensitive and/or personal information) as specified in the State Administrative Manual, Section 5300 to 5365.3; Cal.

  • Personal Information (PI) shall have the meaning given to such term in California Civil Code section 1798.29.

  • All workforce members who assist in the performance of functions or activities on behalf of DBH, or access or disclose DBH Protected Health Information (PHI) or Personal Information (PI) must complete information privacy and security training, at least annually, at Contractor’s expense.

  • Personal Information (PI): Information that identifies or describes an individual, including, but not limited to, their name, social security number, physical description, home address, home telephone number, education, financial matters, and medical or employment history.
