Examples of Physician/Doctor in a sentence
If the District’s chosen Physician/Doctor requires additional appointment(s) to determine the Employee’s fitness for duty, the District shall pay the costs of the appointment and reimburse mileage as in number 2 above.
If the Physician/Doctor determines the Employee is not fit for duty, the District will not reimburse the Employee for the use of sick leave or pay due to lost time from work.
If the Physician/Doctor determines the Employee is fit for duty and the Employee has no sick leave time remaining, the District will reimburse the Employee for the lost time at their regular rate of pay.
When an Employee’s absence from work begins by the District requiring proof of the Employee’s fitness for duty, the District will pay for the Physician/Doctor appointment.
If the Physician/Doctor chosen by the District is outside the community the Employee works, mileage expense shall be reimbursed at the GSA rate per mile at the time of the Physician/Doctor appointment.
When an Employee’s absence from work begins with a document from a Physician/Doctor stating the Employee is unable to work, the Employee must have a document from a Physician/Doctor stating the Employee is fit for duty and can return to work.
