Medical/surgical services rendered by a legally qualified Physician or surgeon.
Services must be provided by or under the direction of a Physician or surgeon who certifies that the services are medically necessary and consequent to the treatment of the cleft lip or palate or both.
Treatment by a Physician or surgeon in the outpatient department of a hospital will be considered up to the Allowable Expense for such treatment.
Hospital visits by a Physician or surgeon will be considered up to the Allowable Expense for each day of eligible hospitalization.

Upon admission to a Hospital or when consulting a legally qualified Physician or surgeon for major emergency Treatment, OTIP Travel Assistance will confirm to the provider (Hospital, clinic or Physician), that you have the required Provincial Health Plan coverage and OTIP travel benefits as detailed above (subject to the exclusions and limits of the Contract).
The requirements of this provision shall apply to the Contractor and to all subcontractors.
The Physician or surgeon rendering the second surgical opinion may be chosen by the covered individual, provided the Physician or surgeon satisfies the conditions listed in the following “Confirming TMJ Surgical Treatment” section.
