Plan Requirements definition

Plan Requirements means the rules and procedures established by the Plan, as amended by time to time, that establish the conditions to be followed by the Provider and Enrollee during the course of treatment.

Plan Requirements. Under Education Law 3012-d, each teacher must receive an APPR resulting in a single composite effectiveness score and a rating of “highly effective”, “effective”, “developing”, or “ineffective”.
