Plan Skilled Nursing Facility definition
Plan Skilled Nursing Facility means a Skilled Nursing Facility which has a written agreement with Blue Cross and Blue Shield of Illinois or another Blue Cross and Blue Shield Plan or Blue Cross Plan to provide services to you at the time services are rendered to you.
Plan Skilled Nursing Facility means a Skilled Nursing Facility which has a written agreement with Blue Cross and Blue Shield of Illinois or another Blue Cross and Blue Shield Plan or Blue Cross Plan to provide Covered Services to participants in the Blue PPO benefit program at the time Covered Services are rendered, or is designated as a Participating Provider by any Blue Cross and/or Blue Shield Plan at the time that Covered Services are rendered.
