Medical Necessity Covered Services must be Medically Necessary, except for Clinical Preventative Care Services.
Under Preventative Care in the City of Rapid City Group Health Plan, the following components of a routine physical examination per benefit year are covered at 100%.
Effective February 28th, 2017, yearly maximums in the Plan shall be as follows: • Preventative Care – $1850*; • Basic Plan – $1850*; • Major Plan – $1850*; • Orthodontic Plan – $2250.
There is no cost sharing for Essential Health Benefit Preventative Care Services.
Although Preventative Care is covered at no charge, an office visit Copay may apply for other Covered Services provided during Your visit.
Special covered expenses Plan pays 100% of expenses for (not subject to deductible): − Pre-admission tests − One routine pap smear, pelvic exam, mammogram, testicular exam, prostate exam, colorectal cancer screening, blood analysis, and urinalysis per calendar year − Second surgical opinion − Lab and X-Ray to $200 Preventative Care per the ACA, including exams and immunizations, contraception, etc.

SSM Medical Group will bill the participant’s insurance for the Preventative Care Services listed below.
Preventative Care| Any Treatment, service or procedure, including but not limited to, physical examinations, Medication, Surgery, inoculations, or laboratory procedures, for the purpose of prevention of Injury or Illness or for the promotion of general health, where there has been no Injury or Illness.
