Recordkeeping Requirements. The vendor shall maintain adequate records of all relevant data with respect to the utilization and/or attempted utilization of M/W/V/DOBEs and shall provide full access to these records to OMWBD upon its request to inspect them. The City may require vendor to submit information in addition to the M/W/V/DOBE Participation Plan and/or M/W/V/DOBE Program Waiver regarding certification or utilization. Such information may include, but is not limited to: • Copies of all executed agreements for each M/W/V/DOBE engaged to satisfy the utilization goals in the performance of the contract; • The name, address, and contact information of the utilized M/W/V/DOBE; • The scope of the work to be performed by the proposed M/W/V/DOBE subcontractor; • The dollar value of the work to be performed or furnished by each proposed M/W/V/DOBE subcontractor and/or joint venture partner; • Acknowledgement and acceptance of the agreement by the M/W/V/XXXX; and • Monthly utilization payment reports submitted with each invoice for payment.
