Referral Facility definition
Referral Facility means any legally qualified and state-licensed intermediate care facility, skilled nursing facility, Hospice, or Hospital that provides services to Members under the orders of a Treating Physician, Affiliated Physician, or Referral Physician when admission is authorized by the Plan’s Medical Director or his/her designee.
Referral Facility means any state licensed intermediate care or skilled nursing facility, Hospice or Hospital which provides services to Members under the orders of an Affiliated Physician/PCP or Specialty Physician. Admission must be authorized by the Affiliated Physician/PCP and the Plan’s Medical Director or his designee.
