Members with (i) a Life-Threatening or Seriously Debilitating Condition or (ii) a degenerative and disabling condition or disease, either of which requires specialized medical care over a prolonged period of time may request that a Specialist or Specialty Care Center assume responsibility for providing or coordinating the Member's medical care, including primary and specialty care.

Prescribed by a licensed Physician for the treatment of a Life-Threatening Condition, or prescribed by a licensed Physician for the treatment of a Chronic and Seriously Debilitating Condition, the drug is Medically Necessary to treat that Condition, and the drug is on the Formulary, if any; and3.
