INDIVIDUALS TO BE SERVED – single adults eighteen (18) years or older who are living 3 with serious mental illness (SMI) and/or co-occurring disorder, who are experiencing homelessness and 4 actively participating in services at a Mental Health and Recovery Services, Adult and Older Adult, 5 County, County contracted outpatient clinic and contracted Full Service Partnerships.
Alameda County Health Care for the Homeless shall support collaborations by sharing available information on scheduling and services among the Street Health Outreach Teams, and the Health Care for the Homeless mobile unit Additionally, Health Care for the Homeless shall ▇▇▇▇▇▇ communication and information-sharing with Alameda County Behavioral Health (ACBH) Crisis Response teams, and ACBH Full Service Partnerships focused on homeless.
Contracted staff shall assist COUNTY discharge planner to secure housing placements (B&C, R&B) for clients who are not currently established and enrolled in COUNTY higher level of mental health treatment services (e.g. Full Service Partnerships).
Contractor shall comply with all State Department of Mental Health (‘DMH”) reporting requirements for Mental Health Services Act Full Service Partnerships including collections using State instruments, maintenance according to State guidelines, and reporting using State processes.
The Exchange Shares allotted and issued to the Service Partnerships shall continue to be subject to the same vesting and other terms and conditions that were applicable to the Service Partnerships’ Subject Shares immediately prior to the Share Exchange Closing, except to the extent acceleration is necessary to permit payment of applicable Taxes.

For Full Service Partnerships or for programs operating under a Master (versus Services As Needed) Contract, Contractor shall give/publish credit in all media transmissions, published materials, or presentations to the community or other interested groups that are supported in part or entirely by this Agreement, to Alameda County Health, Behavioral Health Department.
Each Stockholder, other than the Service Partnerships, shall have executed and delivered a Lock-Up Agreement.
CONTRACTOR will integrate the MHSA programs into core programs in the System of Care by organizing and contracting with providers to initiate Full Service Partnerships (FSP) and by managing and monitoring other MHSA programs and services including prevention programs, innovation programs, care management, housing support programs, community services and supports, workforce education and training.
