Single Point of Contact (SPOC) definition

Single Point of Contact (SPOC) means the entity designated by OHA that coordinates and tracks referrals, and coordinates the provision of services and supports in collaboration with the provider, the CCO, and the CMHP.
Single Point of Contact (SPOC) means the entity that is not directly the certified ACT Program and designated to orchestrate and track referrals. This coordination of services and supports is in collaboration with the Coordinated Care Organizations (CCO), and/or the certified ACT Program.
