Surgical extraction means the extraction of an erupted or im- pacted tooth requiring removal of bone and/or sectioning of the tooth, and including elevation of mucoperiosteal flap if indicated. This in- cludes related cutting of gingiva and bone, removal of tooth struc- ture, minor smoothing of socket bone, and closure.
