  • Attachment A – Professional Services Agreement and Scope of Work Attachment B - Summary of TPA Fees THIS AGREEMENT is made and entered into as of the date of execution by the City of Encinitas, a municipal corporation, hereinafter referred to as "CITY", and AdminSure, Inc, A Third Party Administrator of Workers’ Compensation Program hereinafter referred to as "CONTRACTOR”.

  • Defendants will not oppose an application by Class Counsel for an award of costs, TPA Fees, and up to twenty-five percent (25%) of the Maximum Settlement Fund (or, $120,000) as payment for attorneys’ fees.
