VA Formulary means the National Formulary for medications as listed by the VA’s Pharmacy Benefits Management Services, as updated by the VA and located at http://www.pbm.va.gov/nationalformulary.asp.

  • Formulary means a list of covered prescription drugs provided under this plan. The formulary includes generic, preferred brand name, non-preferred brand name, and specialty prescription drugs.

  • MEDICAL PRESCRIPTION DRUGS are prescription drugs that require administration (or the FDA approved recommendation is for administration) by a licensed healthcare provider (other than a pharmacist). These medical prescription drugs include, but are not limited to, medications administered by infusion, injection, or inhalation, as well as nasal, topical or transdermal administered medications. We will make a determination whether a healthcare service is medically necessary. You have the right to appeal our determination or to take legal action as described in Section

  • Drug means (i) articles or substances recognized in the official United States Pharmacopoeia

  • Prescription means an order for drugs or medical supplies, written or signed or transmitted by word

  • Cannabis wholesaler means any licensed person or entity that purchases or otherwise obtains, stores, sells or otherwise transfers, and may transport, cannabis items for the purpose of resale or other transfer to either another cannabis wholesaler or to a cannabis retailer, but not to consumers.
