Vision care provider definition

Vision care provider means either of the following:
Vision care provider means an optometrist licensed by the board of examiners in optometry or an ophthalmologist licensed by the state board of healing arts.
Vision care provider means a Covered Vision Provider who is acting within the scope of his license to provide Vision Care Services to plan Participants.
