Rheoli • Gweithio'n annibynnol ac fel rhan o dîm i gydgysylltu pob rhaglen Ymyrraeth Iechyd o fewn ardal gymunedol ddynodedig. Gweithio mewn partneriaeth â sefydliadau statudol a gwirfoddol i gydgysylltu cyfleoedd Ymyrraeth Iechyd a fydd o fudd i ddinasyddion sydd, neu sydd mewn perygl o roi mwy o alw ar y system iechyd a gofal cymdeithasol. • Bydd y Mentor Ffyrdd o Fyw Egnïol yn gyfrifol am reoli llwyth achosion cleientiaid a bydd yn gweithredu strategaethau ar gyfer yr unigolion mewn cyd-destunau un-i-un a grŵp, gan weithio'n annibynnol ac fel rhan o dîm. Job Purpose | • The Active Lifestyle Mentor will have responsibility of planning, coordinating and delivering Health Interventions in a designated area or community, with an overall objective of contributing to an improvement in the health & wellbeing of the citizens of Ceredigion. • To be responsible for establishing, delivering and monitoring the National Exercise Referral Scheme (NERS) in a designated geographical area. • To support the Wellbeing Centre Service in providing focussed and universal provision of physical activity opportunities, including centre based and outreach programmes, throughout the county and in partnership with key statutory and voluntary stakeholders |

Overview of the Through Age and Wellbeing Integrated Services Model | The Through Age and Wellbeing Integrated Services Model is a new way of meeting people’s needs by ensuring that the right people are in place to make the right decisions at the right time. The Through Age and Wellbeing Integrated Services Model will: • be the first point of contact for residents and key partners; • design and deliver early prevention and intervention packages bespoke to individuals’ needs; • decide upon the most appropriate services to deliver care and support in a time-related manner; • monitor and evaluate the impact of support. |
Duties and Responsibilities |
Managerial • Working independently and as part of a team to coordinate each Health Intervention programme within a designated community area. Work in partnership with statutory and voluntary organisations to coordinate Health Intervention opportunities that will be of benefit to citizens who are, or are at risk of placing greater demand on the health and social care system. • The Active Lifestyles Mentor will have responsibility to manage a caseload of clients and will implement strategies for the individuals in both one-to-one and group contexts, working independently and as part of a team. Tasks will include; cash handling, opening and closing facilities and booking and managing external venues. • To have responsibility for individual CPD is prioritised and all certified qualifications within their designated remit are renewed and validated according to the awarding body. E.g. Cardiac rehab BACPR, Pulmonary Rehab, PSI Falls Prevention, Cancer Rehab, Stroke Rehab, Mental Health. • Ensure robust National monitoring systems are completed and managed professionally in order to evidence and report qualitative and quantitative data of Health Intervention Clients. To implement strict data and management protocols including GDPR. • The Active Lifestyle Mentor will identify, recruit and train volunteers to assist with certain Health Intervention programmes. Quality and performance management • Develop and deliver a programme of activities which provides measureable physical health and mental wellbeing benefits for adults in a designated Wellbeing Centre or outdoor location. Provisions will include one to one consultations, tailored gym, spin and circuit sessions, water based exercises, specialist seated classes as well as inclusive sessions for adults with disabilities. • Ensure that the citizens of Ceredigion receive access to appropriate information and support throughout their health intervention journey. Staff to ensure appropriate programmes are offered to clients as a continuation to affect long term lifestyle changes, promoting Ceredigion’s Wellbeing and Prevention Plan. e.g. 60+ and Walking for Wellbeing programmes. • Ensure Health Intervention provision reflects the needs of Ceredigion citizens, developing consultative mechanisms to involve citizens in service planning, development and evaluation. Communication • Liaise and coordinate with referred NERS Clients, working closely with the Primary Care referring partner. e.g. GP, Physiotherapists, dietitian and other health care professions in order to identify appropriate safe pathways for individuals who would benefit from the scheme. The needs of the clients will be measured and assessed by the Active Lifestyle Mentor in order to plan and deliver the most effective and supportive provision for them. Functional • Lead and coordinate both focussed and universal Health Intervention provision. The Active Lifestyle Mentor will lead on events, activities and projects to support their community remit. The Officer will work closely with Ceredigion Community Connectors and Wellbeing Centre staff to ensure provisions are appropriate, fit-for-purpose and meet the needs of the clients on Health Intervention programmes. • Identify a range of opportunities to be physically active within an appropriate social or physical activity groups/opportunities including develop social groups for carers and parents. • Ensure that appropriate safeguarding procedures are adhered to professionally. Lifestyle Mentor has a duty to report and respond to any concern in accordance with Ceredigion Safeguarding Policy. • Ensure that all appropriate H&S procedures and policies are adhered to professionally. The Active Lifestyle Mentor has responsibility to ensure that all provisions are planned and coordinated safely and carry out regular dynamic risk assessment according to specific venues and sessions. • To have knowledge and awareness of the relevant National and Local developments to support the appropriate implementation of such initiatives for the benefit of Ceredigion Residents. • To have responsibility for collecting individual session fees and liaising with Wellbeing Centre staff to ensure all income is accounted for and proper banking procedures are followed. • Representation at key meetings as and when required, in PCC service areas and regions Health intervention forums. • Undertake any other duties relevant to the role as required by the Health Intervention Coordinator and Team Manager for Through Age physical Activity & Play Manager. | |

Required Experience | • Minimum of 3 years’ experience in Health intervention. • Minimum of 3 years’ experience in Leisure Services / Physical Activity field. • Experience of a service role in a relevant individual or team and be able to demonstrate achievement in influencing successful performance and/or improvement. • A good understanding of legislation and regulation as it relates to Health and Older Adults, including Physical & Mental Health. | ||
