Diben y Swydd | Bydd Gweithwyr Cymorth i Deuluoedd Braenaru Ceredigion yn gweithio gyda theuluoedd agored i niwed sydd ag anghenion cymhleth, gan gynnig gwasanaethau cymorth ac xxxx dwys i rieni sydd â phlant 0-7 oed. Byddant yn gweithio mewn xxx amlddisgyblaethol a fydd yn cynnwys staff yr Awdurdod Lleol, staff GIG a staff o'r Trydydd Sector er mwyn: • Darparu rhaglenni 1:1 i hyrwyddo iechyd a lles corfforol a chymorth i blant a rhieni yn y cartref gydag arweiniad a mentora ynghylch bwydo, sterileiddio, golchi, diddyfnu, diet a maeth, hyfforddiant defnyddio'r toiled, patrymau amser gwely, diogelwch, patrymau yn y cartref a sgiliau ffordd o fyw i rieni a phlant. • Darparu rhaglenni a strategaethau 1:1 a grŵp a argymhellir er mwyn gwella hunan-effeithlonrwydd rhianta teuluoedd. Gellir darparu cyrsiau grŵp mewn partneriaeth â sefydliadau statudol a'r Trydydd Sector lleol. • Darparu rhaglenni a strategaethau 1:1 a grŵp a argymhellir er mwyn cynorthwyo datblygiad sgiliau iaith, lleferydd a chyfathrebu. • Cynorthwyo teuluoedd a phlant i feithrin sgiliau a gwella'u cydnerthedd i ymdopi'n dda gyda'r sialensiau a'r pwysau y maent yn eu hwynebu. • Cyfrannu at xxxx a lleihau effaith Profiadau Xxxxxxxxx yn ystod Plentyndod (ACEs). • Annog teuluoedd – yn enwedig y rhai anodd i'w cyrraedd – i ymgysylltu â'r gwasanaethau a'r cymorth a gynigir iddynt gan asiantaethau yn eu cymunedau lleol, gan leihau effeithiau tlodi a goresgyn anghydraddoldebau. • Galluogi pob teulu i wireddu eu potensial yn llawn. |

• Cydweithio'n agos gydag Ymwelwyr Iechyd, gweithwyr gofal plant, therapydd iaith a lleferydd, darparwyr gofal dydd, a staff eraill o'r Bwrdd Iechyd, y Gwasanaethau Cymdeithasol, Addysg, Cymdeithas Gofal Ceredigion, CAB a mudiadau gwirfoddol. cyfrinachedd a chadw cofnodion • Sicrhau y cedwir cofnodion perthnasol yn unol â Safonau Cenedlaethol a Pholisi Lleol gan gynnwys cynnal cyfrinachedd, diogelu data a rhannu gwybodaeth, gan roi sylw arbennig i sicrhau caniatâd gwybodus unigolion a theuluoedd trwy gydol y gwaith. • Cynnal rhaglenni gwaith mewn cartrefi teuluoedd i ffwrdd o'r swyddfa. Gall yr amodau mewn rhai cartrefi fod yn anrhagweladwy ac yn anodd. Efallai y bydd rhai teuluoedd xxx xxxx dipyn o straen ac efallai y byddant yn dangos ymddygiad neu agweddau heriol a fydd yn gofyn am amynedd a medr er mwyn eu rheoli'n effeithiol o ddydd i ddydd. Job Purpose | Ceredigion Pathfinder Family Support Workers will work with vulnerable families with complex needs offering intensive prevention and support services to parents with children aged 0-7 years. They will work within a multi-disciplinary team, made up of Local Authority, NHS and Third Sector staff to: • To deliver 1:1 programmes to promote health and physical wellbeing and support to children and parents in the home with guidance and mentoring on feeding, sterilising, bathing, weaning, diet and nutrition, potty training, bedtime routines, safety, household routines and lifestyle skills for parents and children • To provide 1:1 and group based recommended programmes and strategies to enhance families’ parenting self-efficacy. Group courses may be delivered in partnership with the local Third Sector and statutory organisations. • To provide 1:1 and group based recommended programmes and strategies to support the development of speech, language and communication skills • To support families and children to develop skills and enhance their resilience to cope well with the challenges and pressures that they face. • To contribute to the prevention and reduction of the impact of Adverse Childhood Experiences (ACEs) • To encourage families – particularly those who are hard to reach - to engage with the services and support offered to them by agencies in their local communities, reducing the effects of poverty and combating inequalities. • To enable all families to reach their full potential. |

Duties and responsibilities | • To proactively engage and assist children and their parents and carers with emerging difficulties including families where there is identified developmental delay or adverse experiences. To undertake bespoke structured programmes to address the identified issues. • To deliver 1:1 programmes to promote health and physical wellbeing and support to children and parents in the home with guidance and mentoring on feeding, sterilising, bathing, weaning, diet and nutrition, potty training, bedtime routines, safety, household routines and lifestyle skills for parents and children • To deliver Baby Massage techniques to parents, creating good bonding and early attachment for new-born infants, in order to build positive relationships in a preventative manner. • To work with vulnerable families with complex needs, in order to deliver language development sessions, as directed by the Flying Start Speech Therapist, where speech and language difficulties have been identified with pre-school children. Adapting language and vocabulary strategies, delivering a range of techniques that parents can continue in the home environment. • To promote the use of the Flying Start childcare settings to families with children aged 2-3 years and accompany children with their parents (on occasion) to settings for initial settling-in periods. • To accompany vulnerable families with complex and high needs to a variety of venues under the direction of Flying Start Health visitors, including health clinics, parenting groups, family centres, hospital appointments, antenatal |
clinics and groups. • To work closely with Health Visitors, child care workers, speech and language therapist, day care providers, and other Health Board, Social Services, Education, Ceredigion Care Society, CAB and voluntary organisation staff. This includes regular attendance at staff meetings, FSW group meetings, local Integrated Children’s Centre and other meetings as necessary. • Implement Signs of Safety &Wellbeing by identifying and promoting positive aspects of family life, enabling them to address worries and assist them to achieve positive outcomes including improved family relationships, improved physical and emotional wellbeing and improved social connections including learning, training and employment opportunities. • Work directly and consistently with children and their families to contribute to delivery of the strengths based care and support by establishing a clear focus with the parent and help them to reach their goals within the plan. • Maintain regular contact with family members in line with Healthy Child Wales and Flying Start Delegation protocol and timescales in order to assist with communication, build trust and progress action plans. • To promote the Welsh Language and opportunities to develop and use it with families • Participate in multiagency planning as part of a multi-agency team alongside the family using Signs of Safety & Wellbeing methodology to assist with the delivery of agreed outcomes. • Deliver bespoke planned and agreed parent programmes, using a strengths based approach and activities as identified in Parenting in Wales Guidance (WG2014) including one to one structured support when required to assist development of the families understanding, skills and resilience to face pressures and challenges. • Work in conjunction with multi agency team members and Family Centre Network to ensure delivery of termly group based interventions including Family Links Nurturing Programme and Helping Me, Helping You, to assist families to engage with and develop social communication within their local community so as to reduce social isolation and promote wellbeing and a sense of belonging. • To keep accurate records in a SOAP format, meeting NHS |
requirements and Health board policies on confidentiality and record keeping. • Ensure relevant records are kept in line with National Standards and Local Policy including maintaining confidentiality, data protection and information sharing with particular attention to ensuring informed consent of individuals and families throughout the work. • Carry out programmes of work in families homes away from the office base. Conditions in some homes may be unpredictable and difficult. Some families may be in highly stressful situations and may have challenging behaviour or attitudes that will require patience and skill to manage effectively on a daily basis. Home visits involve a level of risk, which is managed via electronic calendar, diaries and checking in arrangements as well as use of mobile phone and alarm systems. • Participate in relevant training and development forums to ensure shared understanding of systems and processes that assist with achievement of corporate priorities and Public Service Board outcomes. • Contribute to delivery of support in accordance with relevant National Occupational Standards in Health, Social Care, Early Years and Child Care. (See website for Care Council Wales). • Ensure that statistical information required by the Pathfinder Officer in regard to service user outcomes is available to meet necessary guidelines, monthly outcome reporting and performance indicators. • The post holder will also be required to carry out physical duties when setting up and clearing away after running groups. • Some light cooking duties while running group sessions. • To maintain the hygiene of equipment used by children and their families • Ensure information is stored and managed in line with the local protocols for information sharing (Wales Accord for Sharing of Personal Information). • Contribute to delivery of a responsive bilingual service in Welsh and English. • Develop knowledge and skills, including all County Council mandatory programmes, in line with individual yearly |
appraisal. • Adopt a flexible attitude to the duties of the post to meet the needs of families including in the evening or weekends as required. | |
Accountability | • To attend regular supervision with the Health Visitor in order to receive and update delegated bespoke packages of care for the children and families in their care, following Flying Start’s Delegation Protocol. • To work to the guidance of the health visitor in keeping records in health board case notes to Hywel Dda University Health Board standards. • To attend group supervision, yearly performance planning and appraisal as well as Learning Sets with the Flying Start Lead Family Support Officer. • To be accountable to the Flying Start Parenting Officer for group work and other activities. • To ensure that the necessary paperwork, e.g. engagement group evaluations and attendance registers are completed and returned to main office. • To be motivated and able to organise own workload from day to day – booking appointments, preparing and delivering activities and programmes within agreed timeframes and to set standards. • To adhere to Safeguarding arrangements as set out in All Wales Chid Protection Procedures. • To adhere to Equality Act 2010 and United Nations Human Rights of Child 1991. |

Experience required for the post | • In depth knowledge and experience of working with parents and cares. • Knowledge of All Wales Child Protection policies and procedures. • Training in family and/or parenting interventions such as Family Links or Incredible Years. • Understanding of the issues parents face, including parental separation and conflict and emotional and mental health issues. • Experience of working with projects requiring close evaluation • Knowledge of child development and contribution to supporting families to provide a positive experience for their children. • Experience of partnership or multiagency teamwork. |
