ข้าพเจ้า ผู้ล งนามท้ายเอกสารฉบับนี้ ในฐานะที่เป็นบุคลากรของคณะแพทยศาสตร์ มหาวิทยาลัยสงขลา▇▇▇▇▇▇▇▇ มีข้อตกลงดังนี้
By signing below, I, as an employee of the Faculty of Medicine, Prince of Songkla University, agree to comply with the following statement:
1. ไม่เปิดเผยข้อมูลส่วนบุคคล▇▇▇▇▇▇รับรู้ รับทราบจากการทํางาน แก่บุคคลใด ๆ ▇▇▇▇▇▇เกี่ยวข้อง
1. I shall not disclose any Personal Data obtained during work to any other person or
entity.
2. การเก็บรักษาเอกสาร บันทึก ข้อมูล หรือวัตถุอื่นใดที่ปรากฏข้อมูลส่วนบุคคลของบุคคลอื่นอยู่ ต้อง
▇▇▇▇▇▇▇ในสถาน▇▇▇▇▇▇เหมาะสม มิดชิดปลอดภัย ป้องกันการเข้าถึงของบุคคลอื่นใดซึ่งมิได้มีหน้าที่เก็บ ดูแล รักษา ข้อมูลเหล่านั้น
2. I shall keep documents, records, or any materials containing Personal Data in a secure place and ensure adequate protection to such Personal Data.
3. ป้องกัน ดูแล รักษาข้อมูลบัญชีชื่อผู้ใช้งาน และรหัสผ่านของตน มิให้ผู้อื่น▇▇▇▇▇▇▇
3. I shall not share or disclose my login information, including user names or passwords, to anyone.
4. ไม่สําเนาหรือทําซ้ําซึ่งข้อมูลส่วนบุคคลของผู้อื่น แม้เพียงส่วนหนึ่งส่วนใดหรือทั้งหมด เว้นแต่เพื่อ ปฏิบัติการตามภาระงาน หรือตามหน้าที่
4. I shall not copy or reproduce any part or all of the Personal Data except as required to accomplish the intent of work or duties.
5. ไม่นําเอกสารข้อมูลส่วนบุคคลออกไปจากคณะแพทยศาสตร์ มหาวิทยาลัยสงขลา▇▇▇▇▇▇▇▇ เว้นแต่ กรณีจําเป็นต้องเปิดเผยข้อมูลส่วนบุคคล เพื่อปฏิบัติ▇▇▇▇▇▇▇▇ กฎหมายหรือคําสั่งศาล โดย▇▇▇▇▇▇ให้ ผู้บังคับบัญชาทราบภายใน 2 วันทําการ▇▇▇▇ นับแต่ได้รับแจ้งคําร้องขอเอกสารดังกล่าว
5. I shall not remove any materials containing Personal Data from the premises of the Faculty of Medicine, Prince of Songkla University. In case if the disclosure of Personal Data protected by contract, law or court order is required, I shall inform the supervisor within two
(2) business days starting from the day of receiving such disclosure request.
6. การนําข้อมูลส่วนบุคคลภายในคณะแพทยศาสตร์ มหาวิทยาลัยสงขลา▇▇▇▇▇▇▇▇ไป เพื่อเป็น กรณีศึกษา การเรียนการสอน การทําวิจัย ต้องทําตาม▇▇▇▇▇▇▇ ข้อบังคับ แนวปฏิบัติของคณะแพทยศาสตร์ มหาวิทยาลัยสงขลา▇▇▇▇▇▇▇▇
6 . The use of Personal Data for the purposes of case studies, education, or research studies must comply with rules, regulations, and guidelines of the Faculty of Medicine, Prince of Songkla University.
ข้าพเจ้ายอมรับเป็นหน้าที่ที่ต้องปฏิบัติตามข้อตกลงดังกล่าว แม้ว่าการปฏิบัติหน้าที่ในฐานะบุคลากร ของข้าพเจ้าจะสิ้นสุดลงด้วยประการใด ๆ
I hereby agree to comply with the Confidentiality Agreement and agree that my obligation bound by this agreement shall continue even my duties as an employee has finished.
