申請須知
1. 每位申請人只須填寫一份申請表，重覆申請者將不獲受理。
2. 申請人提供之個人資料必須真實、準確、有效及完整。
3. 本平台有權按需要及服務安排，將會員選配予指定協作伙伴作跟進及聯絡。
4. 會員必須遵守本平台的規則及本平台職員的安排。
5. 申請人於遞交申請表格前，必須細閱《樂齡科技平台會員的條款及細則》。
重要聲明
凡申請成為本平台會員之人士，敬請仔細閱讀下列條款。一經成為會員，即視作已接納下列條款內容，並將受其約束。
定義
在本條款中，除非內文特別規定，否則下列字詞具有以下涵義：
1. 「本平台」指樂齡科技平台。
2. 「本條款」指《樂齡科技平台會員的條款及細則》。
你的權益
1. 本平台不斷發掘更多的服務資源，不時作出資訊分享，讓樂齡科技平台會員全面接觸本平台服務，掌握最新資訊。
2. 本平台對平台中提及的公司所提供的額外服務概不負責。如有任何爭議，本平台保留最終決定權。
更改資料
會員如更改電郵地址、聯絡電話等個人資料，必須儘快通知本平台職員（電話：
0000 0000 或電郵：XXXxxxxxxx@xxxxx.xxx.xx）。
退出及終止會籍
1. 如會員不符合本平台會員資格，其會籍將自動撤銷。
2. 會員有權申請退出本平台，並須通知本平台職員（電話：0000 0000 或電郵： XXXxxxxxxx@xxxxx.xxx.xx）。會籍一旦終止，退會會員不能再享用會員之權益。請連同以下資料通知本平台職員:
i. 會員姓名
ii. 會員機構名稱
iii. 會員登記電郵
iv. 退出原因
3. 若會員提供任何非真實、不準確、失效或不完整之資料，或本平台持有合理
理由的情況下，懷疑其登記資料之準確性，則本平台有權暫停或取消其會籍。
4. 會員如嚴重違反本平台規定或未能履行本條款內容，均視作違約論。本平台可自行宣佈暫停或撤銷其會籍及所有權益。
其他
1. 本平台保留隨時修改本條款之權利，並以本平台認為適當之方式，事先通知會員。唯通知一旦發出，不論會員接獲與否，均視作已知悉論。於此等修訂生效後，倘會員仍保留會籍，將被視為接受及同意此等修訂。
2. 本平台發予會員之任何通知，無論經電郵或以本平台認為適當的方式，即視為已向會員發出通知。

個人資料使用守則
「樂齡科技平台」盡力遵守及執行《個人資料（私隱）條例》中所列載的規定，確保儲存的個人私隱及個人資料處理得到充份保護，準確無誤及妥善儲存。為確保你能充分了解向平台提供個人資料的準則，請細閱以下指引。
收集資料目的及使用準則
1. 本平台收集閣下的個人資料，只用以處理閣下的入會申請，以及獲接納入會後的會籍管理事宜。
2. 本平台將依照在收集資料時所說明之目的去使用閣下提供的個人資料。
3. 向本平台提供個人資料純屬自願，如閣下未能提供足夠個人資料，本平台可能無法有效處理閣下的申請。
4. 本平台可能使用閣下提供的聯絡人的個人資料 (包括並不限於姓名、職位、電話、電郵、所屬機構名稱)，以便本平台與閣下通訊。
查閱及更新個人資料及申請停止使用個人資料作推廣之用途
除了《個人資料（私隱）條例》規定的豁免範圍外，閣下有權就本平台備存有關閣下的個人資料作出查閱、更改及提出停止使用有關的個人資料作推廣用途的要求，但已達成使用目的後而刪除的個人資料除外。
如閣下不欲本平台使用你的個人資料作上述用途，請以郵寄/傳真/電郵形式通知本平台。

Gerontechnology Platform Membership Terms and Conditions
Membership Application Guideline
1. Applicants shall submit only ONE application. Multiple or suspected multiple applications will be rejected.
2. Applicants shall provide true, complete, valid and accurate personal information.
3. Gerontechnology Platform (The Platform) reserves the right to refer members to particular collaborative partners for follow-up and connection based on the need, appropriateness and service arrangements.
4. Members shall follow the regulations of the Platform and the arrangements of the staff of the Platform.
5. Applicants must read the “Gerontechnology Platform Membership Terms and Conditions” before submitting the membership application.
Important Message
Applicants shall read the below terms in details. Once accepted as member, the person shall be regarded as accepting the terms agreement and bounded by the terms.
Definitions
In this term, unless the context otherwise requires, the following wordings shall have the following meanings:
1. “The Platform” refers to Gerontechnology Platform
2. “Terms” refers to Gerontechnology Platform Membership Terms and Conditions
Your Benefits
1. The Platform shall continually look for more resources and share information from time to time, so members of the Platform can enjoy the Platform’s service and the latest information.
2. Platform takes no responsibility for the extra services offered by the external parties mentioned in the Platform. In case of any disputes, the decision of the Platform shall be final.
Changes of Personal Particulars
Members shall notice staff of the Platform for any changes of your personal particulars by sending email to XXXxxxxxxx@xxxxx.xxx.xx or calling the hotline on 0000 0000.
Membership Termination & Withdrawal
1. The Platform has the right to terminate membership if a Member does not fulfill the requirements.
2. Member has the right to apply for membership withdrawal from the Platform. You must notice staff of the Platform by sending email to XXXxxxxxxx@xxxxx.xxx.xx or calling the hotline on 0000 0000. Once the membership is withdrawal from the Platform, the withdrawn members shall not enjoy the member benefits. Please notice the Platform’s staff with the following information:
i. Member name
ii. Member Organization
iii. Member registered Email
iv. Reason of withdrawal
3. Should a Member provide any false, inaccurate, invalid and incomplete information, or the Platform shall reasonably suspect the accuracy of the information provided, the Platform reserves the right of suspension or termination of the Membership.
4. If a Member violates the Platform’s terms or unable to fulfill the requirements of the terms, the Platform reserves the right of suspension or termination of the Membership and its benefits.
Others
1. The Platform reserves the right to change, add, delete and/or amend, without notice, at any time and from time to time any provisions of these Terms and Conditions. After the amendment commences, members are regarded as accepting and agreeing the amendments if members continues their membership.
2. Any notifications sent by the Platform through email or any appropriate channels, are regarded as notifying the members.

Statement on Use of Personal Data
Gerontechnology Platform (The Platform) undertakes to comply with the requirements of the Personal Data (Privacy) Ordinance to ensure that personal data kept are accurate and securely kept. To ensure you are well informed of the personal data as collected, please read this Statement with care.
Purpose of Collection and Guideline for Use of Personal Data
1. Provision of personal data is for processing membership application and for
business related to membership management upon admission to the Platform.
2. The Platform will use personal data collected from data subject for the purposes for which it was collected.
3. To provide personal data to the Platform is on a voluntary basis. If you do not provide sufficient personal data required for related purposes, the Platform may not be able to process your application.
4. The Platform may use the personal data of the contact person provided by you (including but not limited to name, position, telephone number, email, name of organisation) for the purposes of providing you with information of the Platform.
Access to and Correction of Personal Data and Request for cessation of using Personal Data for Promotion Purposes
Apart from the exemptions provided under the Personal Data (Privacy) Ordinance, you are entitled to access and correct your personal data held by the Platform, and request the Platform to cease to use the personal data for promotion purposes. However, it will not include the personal data deleted after the achievement of the purpose.
If you object the Platform to use your personal data for the purposes as stated above, please notice us by mail/fax/email.
