Application Form的定义

Application Form means an application for the Automatic Add Value Service whether this is (i) an Octopus Automatic Add Value Service Application Form, (ii) a Personalised Octopus Application Form or (iii) any other form containing an application for this service;

Application Form means the form whether electronic submitted via the Exhibitor Online Platform or submitted in hard copy by which the Exhibitor applies to exhibit at the Exhibition.

Application Form means the GREEN application form in agreed form to be used in connection with the Hong Kong Public Offering;

• Valuation Currency means the currency specified by you in the Application Form for valuation of the Plan.

• Please send Deposit Slip / Transaction Record together with the Application Form to insurance company by post, fax or e–mail at least 15 DAYS before trip commences.

• By signing the Application Form or by electronically submitting your application on Phillip Futures’ Website or, if applicable, via a mobile application you confirm that you will accept the terms of the Agreement as applying to all such transactions as a condition of your application.

• The insured person must have declared to us on the Application Form any and all known pre-existing medical conditions (as defined).

• The insured person must immediately inform us of any change in the information given on the Application Form, in particular relating to the insured person's address or country of residence, the birth or adoption of a child or any other change involving the insured person's dependant(s).

• These Terms and Conditions together with the Application Form and the enclosed Plan Schedule and any future endorsements (collectively the “Agreement”) constitute the entire agreement between you and us relating to the transactions contemplated by this Agreement and supersede and extinguish all previous agreements between the parties relating to these transactions.
