Twelve Month Period. The 12month period measured forward from the date an Employee's first FMLA leave begins. An Employee would be entitled to 12 weeks of leave during the 12month period beginning on the first date FMLA leave is taken. The next 12month period would begin the first time FMLA leave is taken after completion of any previous 12month period.

Twelve Month Period. The twelve (12) month period for the purposes of the FMLA shall be measured backward from the date a staff member uses any Family and Medical Leave Act Leave.

The request must include the beginning and ending dates of the leave and must be requested no later than thirty (30) calendar days after the birth of the child.
