This notice is provided under the authority of Connecticut General Statutes §9-612(g)(2), as amended by P.A. 10-1, and is for the purpose of informing state contractors and prospective state contractors of the following law (italicized words are defined later on in the notice).

W-9 Form means a document issued by the United States Internal Revenue Service (IRS) for certain tax purposes. RESPONSIBILITIESThe following agencies have the responsibilities assigned them u nder this policy in addition to those assigned under other policies, State and Federal laws and regulations. D EPARTMENT OF ADMINISTRATION Except as otherwise provided in this policy, the purchasing policy must be implemented and administered by the County Administrator or his/her designee.
