Acknowledgement of Data Used to Obtain Payment Under Federal Program. Group acknowledges and agrees that data furnished by Group to The Plan in connection with the provision of Covered Services under the Agreement, the Blue Choice PPO Addendum and this MA PPO Addendum will be used by The Plan to obtain payment from CMS under the CMS Contract and to support The Plan's participation in the MA and Part D Programs, including submission of bids for renewal of the CMS Contract in future years and for risk- adjusting MA PPO Plan payments from CMS. Furthermore, Group acknowledges and agrees that The Plan and CMS will rely on the accuracy, completeness and truthfulness of any data Group submits to The Plan under the Agreement, the Blue Choice PPO Addendum and this MA PPO Addendum.

PRACTICE acknowledges and agrees that data furnished by PRACTICE to HMO in connection with the provision of Covered Services under this Agreement will be used by HMO to obtain payment from IHFS under the IHFS Contract and to support HMO’s participation in the Programs, including submission of bids for renewal of the IHFS Contract in future years and for risk-adjusting HMO Plan payments from IHFS. Furthermore, PRACTICE acknowledges and agrees that HMO, and IHFS will rely on the accuracy, completeness and truthfulness of any data PRACTICE submits to HMO under this Agreement.
