Acknowledgment of Funding Support. The following statement will be included on the Hospital’s website, on all Publications and, upon request of the LHIN, on any other publication of the Hospital relating to a Hospital initiative: “The [Insert name of Hospital] receives funding from [Insert name of LHIN] Local Health Integration Network. The opinions expressed in this publication do not necessarily represent the views of [Insert name of LHIN] Local Health Integration Network.” Upon request of the LHIN, the Hospital will include a statement in a form acceptable to the LHIN, acknowledging the support of the Province.
