Admission Services. 18 1) CONTRACTOR shall admit individuals who have been determined to meet 19 admission criteria and will have the resident sign an admission agreement describing the services to be 20 provided, resident rights, and the expectations of the resident regarding house rules and involvement in 21 all aspects of the program, including individual and group therapy sessions. 22 2) CONTRACTOR shall complete a thorough behavioral health assessment and 23 psychiatric evaluation within twenty-four (24) hours of admission. 24 3) During the initial seventy-two (72) hours subsequent to admission, residents will be 25 expected to remain on site at all times to ensure integration into the program. After this initial period, 26 resident may be eligible for a day pass to an approved activity, usually an MD appointment or an 27 appointment for housing, etc. Prior to the approved activity pass, the resident must be clinically evaluated 28 an hour prior to departure and immediately upon returning to the facility. The resident must be clinically 29 approved prior to leaving the facility. These clinical evaluations will be clearly documented in the 30 31 4) CONTRACTOR shall obtain or complete a medical history within three (3) days of 32 admission. 33 5) CONTRACTOR shall be responsible for resident’s TB testing upon admission if 34 resident has not completed the test prior to admission to the program. 35 6) CONTRACTOR shall not refuse referrals if CONTRACTOR has available space 36 and appropriate staffing, unless mutually agreed upon by CONTRACTOR and ADMINISTRATOR. 37 // 1 7) CONTRACTOR and resident will together develop a written treatment/service plan 2 specifying goals and objectives, involving resident’s family and support persons as appropriate, and as 3 aligned with a recovery focused, person-centered and directed approach within seventy-two (72) hours of 4 admission. CONTRACTOR shall involve the resident’s family and support persons or document attempts 5 to obtain consent until consent is obtained or the resident is discharged.
