AMBULATORY SURGICAL FACILITY. a Facility Provider, with an organized staff of Physicians, which is licensed as required by the state, and which, for compensation from its patients: a. has permanent facilities and equipment for the primary purpose of performing surgical procedures on an Outpatient basis; b. provides treatment by or under the supervision of Physicians and nursing Services whenever the patient is in the facility; c. does not provide Inpatient accommodations; and d. is not, other than incidentally, a facility used as an office or clinic for the private practice of a Professional Provider.
