Amendment to PHI. Business Associate agrees to make any amendment(s) to PHI in a Designated Record Set as directed or agreed to by Covered Entity pursuant to 45 C.F.R. section 164.526, or take other measures as necessary to satisfy Covered Entity’s obligations under 45 C.F.R. section 164.526. Business Associate shall do so solely by way of coordination with Covered Entity, and in the time and manner designated by Covered Entity.
Amendment to PHI. You agree to notify MTM immediately in the event you receive a request from an Individual to amend or otherwise modify any PHI in your possession or control. In fulfillment of your obligations under 45 CFR 164.526, you agree that, at our request, you will make any amendments to PHI that MTM has directed or authorized pursuant to 45 CFR 164.526.
Amendment to PHI. Within twenty (20) days of a request by Covered Entity, Business Associate shall make any amendment(s) to PHI in a Designated Record Set as directed or agreed to by Covered Entity pursuant to 45 CFR 164.526, or take other measures as necessary to satisfy Covered Entity’s obligations under 45 CFR 164.526.
Amendment to PHI. We will make PHI in a Designated Record Set available to you for amendment by giving you access to the Services where that data is kept. You shall incorporate any amendments to the PHI in accordance with 45 CFR § 164.526.
Amendment to PHI. If the services performed under the Service Agreement require Business Associate to maintain a designated record set, within thirty (30) days of receipt of the Department’s request, Business Associate agrees to make any amendments to PHI, if applicable, as the Department directs or agrees to pursuant to 45 C.F.R. § 164.526.
Amendment to PHI. You agree to notify SAFR CARE immediately in the event you receive a request from an Individual to amend or otherwise modify any PHI in your possession or control. In fulfillment of your obligations under 45 CFR 164.526, you agree that, at our request, you will make any amendments to PHI that SAFR CARE has directed or authorized pursuant to 45 CFR 164.526. Modification of this Agreement may be made in accordance with any change in the applicable HIPAA or HITECH laws.
Amendment to PHI. Business Associate agrees to make any amendment(s) to Protected Health Information in a Designated Record Set that the Covered Entity directs or agrees to pursuant to 45 CFR §164.526 at the request of Covered Entity or an Individual, and in the reasonable time and manner designated by the Covered Entity, and to take other measures as necessary to satisfy Covered Entity’s obligations under 45 CFR §164.526, provided that Business Associate has Protected Health Information in a Designated Record Set.
Amendment to PHI. Business Associate shall, upon receipt of notice from Covered Entity but in no case longer than ten (10) business days, make available PHI in a Designated Record Set to Covered Entity for amendment and incorporate any amendments to the PHI in accordance with 45 CFR § 164.526, or take other measures as necessary to satisfy Covered Entity’s obligations under 45 CFR § 164.526. If any Individual to whom the PHI relates directly requests that the Business Associate make an amendment to such PHI, Business Associate shall notify Covered Entity and provide all relevant information to Covered Entity without unreasonable delay, but in any event within twenty (20) days of such request, necessary for Covered Entity to fulfill the request.
Amendment to PHI. DNAnexus will make PHI in a Designated Record Set available to Xxxxxx for amendment and incorporate any amendments to the PHI, as may reasonably be requested by Xxxxxx in accordance with 42 U.S.C. § 17935(e) and 45 C.F.R. § 164.526. DNAnexus shall notify Xxxxxx within five (5) days of receipt of a request for access to PHI.
Amendment to PHI. HEMR will make PHI in a Designated Record Set available to You for amendment and incorporate any amendments to the PHI, as may reasonably be requested by You in accordance with 45 C.F.R. § 164.526.
