Applicant Notification. The Sliding Fee Discount Program determination will be provided to the applicant(s) in writing and will include the percentage of Sliding Fee Discount Program write-off or, if applicable, the reason for denial. If the application is approved for less than a 100 percent discount or denied, Xxxxx Family Wellness Center will work with the patient and/or responsible party to establish payment arrangements. Sliding Fee Discount Program applications cover outstanding patient balances for six months prior to the application date and any balances incurred within 12 months after the approved date unless their financial situation changes significantly. The applicant has the option to reapply after the 12 months have expired or anytime there has been a significant change in family income. When the applicant reapplies, the look back period will be the lesser of six months or the expiration of their last Sliding Fee Discount Program application.

Upon written request, the following will be provided by the OHRM within ten (10) working days to employees who applied for the position in question:
a. Basic qualification determination;
b. The grouping in which the candidate was placed (i.e., basically, well, and/or best qualified)
c. If candidate was referred or not referred; and d. Name of individual hired. Pursuant to 5 USC 7114 (b) (4), the Union is entitled to request information pertaining to the merit promotion process of any bargaining unit position filled by the Agency covered by this Agreement.

12.1 For the purpose of this Contract, Preferred Assignment positions are U.S. Park Police positions to which no additional pay is attached. There are two categories of Preferred Assignment positions: Station Preferred Assignments and Geographic Area Preferred Assignments.
