The Contractor shall support clients to establish a meaningful connection with an assigned primary care, brick and mortar medical home selected by the client. Those with high needs shall be prioritized with intensive visits and services to improve health and permanent housing opportunities. Contractor shall have assigned medical provider (i.e., Contractor’s Nurse Practitioners (NPs), Physician Assists (PAs), or Medical Doctors (MDs) for the Registered Nurse (RN) to consult when certain clinical scenarios arise and to provide street based care to complex patients with multiple barriers accessing clinic based care who would benefit from targeted and time-limited treatment by a provider in the field.
