Athletic Participation. I further agree that should I use the College’s weight room or any other athletic facility on campus, participate in athletics at the College, participate in intra-mural sports on College teams, or participate in athletics at College events on or off campus, there are certain risks of injury that are inherent to participation in sports and recreational activities. These types of injuries may be minor or serious and may result from my own actions or the actions or inactions of others or a combination of both. I hereby warrant that I am physically fit should I choose to participate in such activities and that I understand that the choice to participate brings with it the assumption of those risks that are a part of athletic or recreational activities. Accordingly, I agree that the College, the University of Toronto, and those for whom they are in law responsible, their successors and assigns, shall not be liable for any injury to my person or loss or damage to my personal property arising from, or any way resulting from, my participation in these activities.

Athletic Participation. Any Student participating in a sports activity is expected to:  Exhibit Xxxxxx-like character at all times…be a good winner and accept losses with humility.  Maintain good grades and be current on all assignments.  Do their personal best in academics and behavior.  Give fully of himself/herself during the practice and games.  Be accepting of his/her mistakes.  Strive to win without placing undue pressure on self or teammates.  Recognize and respect strengths and weaknesses of teammates.  Accept the guidance of coaches.  Respect the decisions of officials and umpires.  Refrain from offensive language and actions.  Athletes must attend all practices and contests unless excused by the coach (teams practice and play after school, on Saturdays, and during some school vacations).  Erratic attendance could be grounds for dismissal from the team. Athletic Participation. If a student is a member of more than one activity that meets after school, participation in the academic activity will take precedence over non-academic activities. Sponsors and coaches may work together to waive this requirement depending on the event.

Athletic Participation. The Interscholastic Athletic Participation Requirement forms must be submitted to the school in order to participate in Interscholastic Athletics (including tryouts). These forms were emailed previously and are also on the school website. Over-the-Counter and Prescription Medication Consistent with NYS and NYC Guidelines for Administration of Medications in School, any student who needs to carry medication, whether prescription or over-the-counter (including ibuprofen and acetaminophen), and who may need to self-medicate during school or any school-related activity, must have the MAF/Self Medication Release Form completed and signed by her healthcare provider and parent/guardian and submitted to the school office. (MAF Form for Prescriptions and SMRF for OTCs are available on the school website health link.) The school office is not permitted to dispense any over-the-counter or prescription medication that has not been provided by the student, stored in the office, and labeled with the student’s name, in addition to submitting an OTC and/or MAF form.
