Award Eligibility. For the 2015-2016 school year only those vehicles meeting the maximum age requirements outlined in item "C" will be considered. Because vehicle model years and school calendar years do not coincide, for purposes of the 2015-2016 school year, a 2014 vehicle will be considered as one year old. Milwaukee Public Schools reserves the right to immediately terminate each and every route on which a contractor is using vehicles that do not meet the age requirements.
Award Eligibility. In determining eligibility for a Transportation Safe Driver Award or a Maintenance/Facilities Department Attendance Award, an employee shall not be penalized, for purposes of time accumulation toward such award, by reason of the first thirty (30) work days of absence per calendar (award) year caused by an industrial injury sustained while on the job. This excusal is limited to one (1) occurrence per calendar (award) year.

I agree to file a FAFSA and TAP application in a timely manner for each academic year to be considered for a CS-LEAP Scholarship renewal. I understand that the amount of my CS-LEAP Scholarship is limited to $10,000 per academic year and that my scholarship award may be adjusted according to my demonstrated financial need.

  • Employee Eligibility For purposes of this section, “eligible employee” shall be defined by the Public Employees’ Medical and Hospital Care Act.

  • Benefit Eligibility For purposes of the Benefit Plan entitlement, common-law and same sex relationships will apply as defined.

  • Program Eligibility The COUNTY shall provide eligibility determination for those persons applying for home repair under this Agreement by using the following factors: 1. The applicant is a resident of the CITY; and 2. The total income for all members of the applicant’s household does not exceed 80% of the median income of the Kansas City metropolitan area, as determined by the Secretary of Housing and Urban Development; and 3. The applicant is the homeowner and must have occupied the property as a primary residence for at least six (6) months; 4. The property to be repaired is within the corporate limits of the CITY; and 5. When required, medical need will be substantiated and documented.

  • Determination of Eligibility The Plan Administrator shall determine the eligibility of each Employee for participation in the Plan based upon information provided by the Employer. Such determination shall be conclusive and binding on all individuals except as otherwise provided herein or by operation of law.
