Benefit Design. Each Stand Alone Dental Plan (SADP) offered by Contractor under the terms of this Agreement shall provide dental benefits in accordance with the Benefit Plan Design requirements as required under this Agreement, and as applicable, any other laws, rules and regulations, including California Health and Safety Code § 1367.005, California Insurance Code § 10112.27, California Government Code § 100503(e), 45 C.F.R. § 155.1065, 45 C.F.R § 156.150, and as applicable, 45 C.F.R. § 156.200(b).
Benefit Design. Carrier shall ensure that each of its SADPs complies with the Benefit Design Standards, including the actuarial value requirements and the pediatric dental component of federally approved EHBs.
Benefit Design. Each QHP operated by Contractor under the terms of this Agreement shall provide essential health benefits in accordance with the Benefit Plan Design requirements set forth at Attachment 2, and as required under this Agreement, and applicable laws, rules and regulations, including California Health and Safety Code § 1367.005, California Insurance Code § 10112.27, California Government Code § 100503(e) and as applicable, 45 C.F.R. § 156.200(b). Notwithstanding the above, QHPs include those defined in 45 CFR Part 155, Subpart K, Section 155.1065(d).
Benefit Design. Carrier shall ensure that:
3.1.1 Each of its QHPs complies with the Benefit Design Standards, including the Cost- Sharing limits, EHBs, and, except for Catastrophic QHPs, Level of Coverage requirements; and

Each QDP offered by Contractor under the terms of this Agreement shall provide the Children’s Dental Plan and Family Dental Plan in accordance with the Benefit Plan Design requirements at 10 CCR 6460 and as identified in Attachment 2, and as required under this Agreement, and as applicable, any other laws, rules and regulations, including California Health and Safety Code § 1367.005, California Insurance Code § 10112.27., California Government Code § 100503(e), 45 C.F.R. § 155.1065, 45 C.F.R § 156.150, and as applicable, 45 C.F.R. § 156.200(b).
Benefit Design. The State reserves the right to determine the benefit designs which best serve the citizens of the State of Vermont. Contractor shall offer on VHC only those products set forth in Exhibit 1 at the Premium rates set forth in Exhibit 1, and the same may be amended on an annual basis in accordance with Section 2.08 hereof.
Benefit Design. The Scheme undertakes to advise the Specialist and the relevant Society/Group, of any changes to the benefit plans or any formulary or protocol changes implemented by the Scheme by giving at least 30 (thirty) days prior written notice thereof;
