Common use of Benefit Forms Clause in Contracts

Benefit Forms. a. The Board shall provide each teacher with an application or enrolment form for participation in the medical, dental, extended health and group life insurance benefit plans. In the event a teacher does not wish to participate in any particular benefit plan where opting out is an option, the application or enrolment form must be so noted by the teacher and kept on file by the Board. b. The Board and Union shall assist employees in obtaining required benefits from the various benefit plans. c. Benefit coverage shall be extended to the end of the next teaching month following a deduction of premiums.
