b. Vendor must sign the attached Goods and Services Agreement prior to execution. No exceptions to the agreement will be considered.

A. Bidder must have the business licenses to perform the services required under the terms of this contract in accordance with local, state, and federal laws at the time of bid submission. Bidder must submit business license(s) to be deemed qualified.
B. Bidder must show continuous engagement in the business of recycling, de-manufacturing, collecting, packing, removing, storing, transporting, shipping and properly managing end of life (EOL) electronic equipment applicable to meet the needs of the contract. Bidder must submit a list of current contracts reflecting similar services to be deemed qualified.
C. Bidder must be a registered small- or large-quantity handler of Universal Waste by the Florida Department of Environmental Protection (FDEP). Bidder must submit registration documentation to be deemed qualified.
D. Bidder must be a registered Mercury-Containing Device Handler by the Florida Department of Environmental Protection (FDEP). Bidder must submit registration documentation to be deemed qualified.
E. Bidder must demonstrate, in written format, that they have the ability to receive, triage, process and store EOL electronic equipment in an enclosed and covered area. Bidder must provide location(s), maps and photos to be deemed qualified.
F. Bidder must be certified by an accredited, independent third-party auditor that they meet specific standards to safely recycle and manage EOL electronics.
