CHANGES TO THE PROJECT WORK PLAN. A. Any modifications to Grantee’s Project Work Plan (PWP) requires prior written approval by HHSC and must only be effectuated by amendment. B. Either Party may identify the need for a change that does not alter the Statement of Work, but makes limited modifications to meet unanticipated needs at any point during the term of the Grant Agreement. C. Changes to the PWP may include: 1. Points of contact; 2. Service delivery area; 3. Outreach and Communication plans; 4. Strategies for providing local approach and personalized support; 5. Participant recruitment and retention strategies; 6. Identification and demonstration of additional outcome measures; 7. Changes to Researched-Based strategy; or 8. Changes to the measurement, quantity and report impact.
