City Contribution Amount Clause Samples
City Contribution Amount. For the period from June 1, 2018 through December 31, 2018, the City’s contribution amount is up to the monthly premium amount of the employee’s eligible coverage level for the ▇▇▇▇▇▇ Permanente HMO plan offered by CalPERS at the basic premium rates designated for the Los Angeles Area. Thereafter, starting January 1, 2019, the City’s contribution amount will return to the monthly premium amount of the employee’s eligible coverage level for the second highest PPO plan offered by CalPERS at the basic premium rates designated for the Los Angeles Area.
City Contribution Amount. For the term of this Agreement, the City’s contribution amount is up to the monthly premium amount of the employee’s eligible coverage level for the second highest PPO plan offered by CalPERS at the basic premium rates designated for the Los Angeles Area. If an employee enrolls in a plan with higher premiums, he will be responsible for the payment to the City on the premium amount in excess of the City’s contribution.
