The Claimant hereby represents, and in connection with the submission of any claim shall certify, that the Claimant has reviewed the information and materials submitted in connection with a claim filed through Trust On-Line with the OCFB Trust and that to the best of the Claimant’s belief, under penalty of perjury, the information submitted is accurate and complete.
